    PROFILE OF CKMB AND TROPONIN I EXAMINATIONS IN PATIENTS WITH CHEST PAIN COMPLAINTS

    Abstract
    Background: Chest pain was chief complaints in emergency department. This complaints could be caused by cardiac or non cardiac. Cardiac cause could be ACS (acute coronary syndrome) thus cardiac enzyme assay is mandatory to exclude cardiac cause in patients with chest pain. The most ordered cardiac enzyme assay are CKMB (Creatinin Kinase MB) and Troponin I. The aim of this study is to know how both assay ordered and how much inter-rater agreement in positive results in patients with chest pain complaints. Method: This study using observative cross sectional design. Subject population in this study were all patients with chest pain complaints who came to JIH hospital in 2014-2015 who ordered CKMB and Troponin I assays by emergency unit doctor. 2 x 2 table was used to measure weight kappa coefficient. Results: 116 subject followed this study, 81 patients did CKMB assay followed by Troponin I assay. 9 patients did CKMB assay only and 25 patients did Troponin I assay only. 74 patients were positive in CKMB assay with median 421 (1-25019), 43 patients were positive in Troponin I assay with median 38 (2-1635}. 2 x 2 table showed that 43 patients/53% were positive in both assay, 7 patients/8,64% were negative in both assay. 31 patients/38,27% were positive in either assay. Weighted Kappa coefficient for this study was 0,913. Conclusions: CKMB and Troponin I were measured in almost all patients with chest pain complaint. There was almost perfect agreement between CKMB and troponin I assay result in patients with chest pain complaints.
