Analisis Biaya pada Pasien Caesar dan Persalinan Normal Rawat Inap Jaminan Kesehatan Nasional (JKN) di RS PKU Muhammadiyah Yogyakarta Tahun 2014
Abstract
The maternal mortality rate (MMR) in Indonesia is still high, despite
declining. Direct causes of maternal death by 90% occurred on during labor and
delivery immediately taxable income. The direct causes of maternal death are
28% of hemorrhage, 24% of eclampsia, 11% of infections, 5% of obstructed labor
and 5% of abortion, so that health financing in Indonesia is increasing from year
to year it is necessary to analyze in any health care financing. Implementation of
clinical pathways is expected to improve patient safety, reduce complications and
lower the cost of patient care. The purpose of this study is to describe the use of
prophylactic antibiotics including the type, dose, frequency and route of
administration, determine the average cost of medical instantly be incurred and
determine the suitability of rates INA-CBGs use of antibiotic prophylaxis in
caesar patients and natural birth hospitalization National Health Assurances (JKN)
in RS PKU Muhammadiyah Yogyakarta in 2014. This study was an observational
study with cross sectional design. Overview of the use of prophylactic antibiotics
for cesarean section in accordance with clinical pathways at 100%. While the
description of the use of prophylactic antibiotics in patients with normal delivery
is not in accordance with the guidelines maternal health services in primary health
facilities and referral. The average percentage of the largest direct medical cost
components incurred by caesarean patients is the cost of services amounted to
60.25%. While the average percentage of the largest direct medical cost
components incurred by patients with normal labor is the cost of services
amounted to 42.20%. The total real cost of hospital patients had negative
difference Rp.310.254.373 caesar against INA-CBGs rates. While the total real
cost of hospital deliveries Rp.8.189.510 normally has a positive difference to the
INA-CBGs rates.
