dc.description.abstractThe Solo-Yogyakarta-NYIA Kulon Progo Toll Road Construction Project Section II Package 2.1. is located on two active faults, namely the opak fault and mataram fault. Based on the soil condition in the project, it tends to be loose sand category, so it can cause liquefaction potential. Like other construction projects, every project has its problems, one of which is construction safety. This research aims to identify potential hazards in bore pile work by considering disaster-prone area factors using the Root Cause Analysis method with Failure Mode and Ef ect Analysis (FMEA) instruments. Risk control ef orts, using FMEA can help identify potential hazards based on failure modes. In this way, appropriate control measures can be implemented. Each job is identified and described based on the Work Breakdown Structure (WBS) principle. Data were collected through observation and interviews, then analyzed by FMEA and verified by OHS Experts. The identification results showed 35 potential hazards originating from various failure modes, with the three main causes being workers who do not understand safe work methods (16 potential hazards), earthquakes due to fault activity (11 potential hazards) and incompetent operators (11 potential hazards). The human factor is the largest contributor to risk with unsafe actions dominating. This research shows that the FMEA method can analyze potential failures and their impacts, so it can be an important tool in bore pile risk control.en_US
