    Abstract
    The construction sector is an important sector that plays a role in the progress of Indonesia, one of which is the development of construction projects. The construction of construction projects cannot be separated from the risk of work hazards, so an Occupational Safety and Health (K3) program is needed to avoid and reduce the number of work accidents among workers. The application of K3 is needed in construction project work, one of which is jetty construction in stopper clamp installation work. The aim of this research is to find out and analyze the roots of the problems that cause work accidents in the work of installing stopper clamps on the jetty upper structure project and to provide appropriate risk control. The data source for this research was obtained by observing the conditions of the accident and interviewing internal project parties. Apart from that, it uses operational standard document data and accident investigation reports as a complement. Then, analyze using the FTA (Fault Tree Analysis) method to trace and analyze various system errors or work accidents (failures) as top events, then narrow down to find the causes and factors for accidents. The results of this research obtained 9 combinations of basic events, namely 2 combinations of environmental factor events, 3 combinations of worker factor events, 1 combination of equipment factor events, and 3 combinations of K3 management factor events. Apart from that, there are ideas for completing risk controls that need to be implemented based on the risk control hierarchy guidelines.
